Thank you for completing your HealthEquity Health Savings Account (HSA) opt-in form. Blue Cross VT will review your request and confirm you’re on an eligible health plan for an HSA. Once confirmed, we’ll notify HealthEquity of your enrollment. If additional information is needed, we’ll contact you.

HealthEquity will confirm your account information and verify your identity. Once confirmed, HealthEquity will mail your welcome materials within 7-10 business days.
