Blue Cross and Blue Shield of Vermont and the Vermont Health Plan are required to notify you of current legal rights and policy limitations that may apply to your coverage with us. The following documents contain important information, and we encourage you to review these materials carefully.

Our notice of privacy practices describes the various ways in which we may use and disclose your protected health information in the course of providing your health benefits.
This document contains several important notices.
- A notice of your rights under the Newborn and Mother's Health Protection Act, which entitles you to certain minimum benefits related to hospital stays and childbirth.
- A notice of your rights under the Women's Health and Cancer Rights Act of 1998, which entitles you to certain benefits for reconstruction surgery related to mastectomy.
- For individuals participating in group health plans, a notice of special enrollment rights which describes the circumstances under which you may be allowed to enroll in your employer's plan or add dependents to your group health plan outside of the normal open enrollment. Please note that if you purchase your group health insurance through Vermont Health Connect, you should consult Vermont Health Connect about the special enrollment periods applicable to those plans.
